The most dangerous fact about cocaine is that nobody who buys it knows what is in it. There is no pure version waiting to be found. By the time powder reaches a person it has been stretched with cheaper compounds, contaminated with industrial chemicals from the way it was made, and, more and more often, laced with fentanyl[1].

That unknowability is the whole danger. A gram sold as high quality is a sales pitch, not a lab result, and the person snorting it is running a chemistry experiment on their own body. Seized cocaine has averaged only around 55 percent actual cocaine, with the rest a shifting mix of fillers and active drugs[2].

What to do:

  • Call 911 first. Tell them where you are and that someone is not breathing. Most states protect people who call to save a life.
  • Give Narcan (naloxone) if it is nearby. Cocaine itself is a heart-and-brain emergency that Narcan cannot reverse, but street cocaine is now often cut with fentanyl, and Narcan reverses that opioid. It cannot hurt someone who does not need it, so give it and repeat after 2 to 3 minutes if there is no response.
  • For a stimulant emergency — chest pain, a seizure, hot skin, severe agitation — still call 911, move the person somewhere cool, and stay with them until help arrives.

Why There Is No Pure Street Cocaine

Purity on the street is a marketing claim, not a measured fact. Cocaine gets cut for one simple reason: a dealer who stretches a kilo with cheaper powder has more grams to sell and more money to keep, so the drug is padded with compounds the buyer never sees[1].

Levamisole Is a Livestock Dewormer That Attacks the Body

The single most common additive in cocaine is one almost no one would take on purpose. Levamisole is a veterinary dewormer, given to cattle and pigs, that was withdrawn from human medicine in the United States around 2000 after reports of serious harm[3]. It now saturates the supply, detected in up to 79 percent of street samples[3].

Levamisole Destroys Infection-Fighting White Blood Cells

Levamisole’s worst trick is turning the immune system against itself. It can trigger agranulocytosis, a collapse in the white blood cells that fight infection, leaving a person open to infections a healthy body would shrug off[6]. Levamisole Kills Patches of Skin

The other signature injury is visible. Levamisole can set off an autoimmune attack on small blood vessels, an ANCA-associated vasculitis that shows up as dark, dying patches of skin on the ears, nose, cheeks, and limbs[7]. In severe cases the tissue simply dies, and the same contaminant has been tied to lasting damage in the brain of long-term users[8].

Fentanyl Is the Deadliest Thing in Cocaine

Of everything hidden in a bag of cocaine, fentanyl is the one that kills fastest. The overdose crisis has entered a fourth wave in which powerful synthetic opioids keep turning up inside stimulants, and fentanyl-involved cocaine deaths have climbed sharply[9]. The danger is brutally simple. A person expecting a stimulant gets an opioid instead.

How Cocaine Is Really Made

Cocaine does not come out of the ground as white powder. It is wrung out of coca leaf with a battery of industrial chemicals, the kind stocked at a hardware store or a gas station, and it is the residue of those chemicals that matters here, not the method[5].

Turning leaf into coca paste and then into powder relies on solvents and reagents never meant to touch a human body. The Chemical Residue Rides Into the Powder

A clandestine lab has no inspector and no rinse cycle, so traces of those solvents and acids do not fully wash out[12].
